Here is where I will post the top 10 lists for this site, but I need your input.

Let's start with The  Top 10 Favorite Character Mistakes.

Submissions for Favorite Character Mistakes

Shasa-1st Level Priestess of Life:
The Group had captured some assassins and was interrogating
(read torturing) them to find out where to find the leader. Shasa decided
she didn't like the interrogation techniques and so decided to set fire to
the group wagon. Unfortunately the wagon contained roughly 500 pounds of
black powder, 25 flasks of Grecian fire, 50 pounds of nails and various
other sharp pieces of shrapnel. After the blast, approx. a 300 yard square
circle of forest was gone and she had left a 30 foot crater in the middle of
a major trading road, and a massive forest fire was raging. She had to spend
the next week healing all of the animals that had been caught in the fire.
She is still working off the 5000gp fine levied on her by the king.

I've got three major blunders for you:
1. Party containing: Fauste lv 8 fighter, Will lv 9 bard, Tass lv 9 thief. After having taken on the job of defeating a group a bandits that had been plaguing a small religious order the party set out to crash the evil doers. After having promptly dispatched the unfortunate individuals the party cut off the leader's head to return it as proof of a job well done. They then proceeded to return to the head cleric to receive payment, unfortunately the cleric had another idea as to what taking care of meant, and was shocked when the pcs announced that they had killed the bandits and promptly expelled them form his office. About a day later while walking in the woods Will for some reason asked who had the bandits head, the pcs looked at each other and wondered who actually had it, when Tass suddenly realised: "Oh crap!!! I left it on the cleric's desk...."
2. The second incident involved the same party: While exploring the castle of a vampire lord the pcs came across a large hall with 6 statues and a large symbol (read X) inscribed on the carpet. Tass realised that the statues were probably some sort of control system and decided to go try to activate them, mean while Fauste decided to go stand on the symbol. Ah few minutes later Tass figured out how the trap door system worked and opened up a 15 foot pit right under Fauste.
3. Same party once again: While exploring an underground lair the pcs came across a section of the cave whose walls were completely covered in florescent moss. When suddenly Will disappeared. Tass hurried to where he had vanished only to disappear himself, mean while Fauste who was admiring the moss suddenly realised that he was alone, he moved in the direction where his friends had vanished, and about where the had disappeared he found a hole. Forgoing the usually formality of calling to see if his friends were alright he jumped right in nearly crushing tass when he hit the ground for 10 damage ( 6 from falling + 4 for absolute stupidity).
